License Commitment Form
Upon submission of the Agreement Letter with Horizon, Districts will identify how many licenses under each model they intend to use during the 2024-25 school year by completing this License Commitment Form. Execution of the Agreement Letter indicates a commitment by the District to purchase said licenses. Horizon will invoice for these initial licenses in early September. Additional licenses can be purchased throughout the contract period upon written request of District via submission of an additional License Commitment Form. Payment for additional licenses shall be made within 45 days of invoice.
